Preparation for the Lord's Supper

Please describe your process for self-examination before the Lord's Supper. If you're a pastor, does your church have any materials to help walk your communicants through this?
 
We first hear a sermon preached leading up to it, then have some scripture read dealing with communion, then hear some exhortations, rules, and promises dealing with the topic, then pray, and then take the elements. It is always accompanied with the Word. We do this every week for the evening service. We are told to come in a worthy manner, confess sin that needs to be confessed before coming, etc. We are told to not come if we are living in unconfessed sin, but to come if we are fighting sin and looking for grace.

Thomas Watson's book called the Lord's Supper is very good. I hope this is helpful.
