Help Wanted: A Historical Theology of "Confirmation"

Greetings,

I was curious to learn more about the doctrinal development of confirmation and whether or not this rite is essentially analogous to what my Presbyterian friends call their communicant member class.

I would also be interested in learning how the rite may have developed coming out of the Reformation and whether there are any notable differences in practice among the various Reformed denominations today.

Has there ever been any points of debate or controversy throughout church history on this topic?

If willing to share, I'd love to hear what your church's process is for admitting people as communicant members.
