Creeds & Confessions Booklet - Greenville?

Hello all, wondering if someone can help me track down a resource. Several years ago I was given a booklet that fits the description below, it has recently been misplaced and it is a significant loss - it is a very useful tool for meetings and visits in particular. Is anyone familiar with this resource and/or know where another copy can be obtained?

• Contained: Ecumenical Creeds, Westminster Standards, Three Forms of Unity
• Spiral bound
• Tabbed sections
• Dimensions similar to a paperback book
• Was possibly printed by Greenville Seminary

I'm quite certain that was printed by Mid-America Reformed Seminary.
