I have always taken it to mean that "hell" is (and has always been) the holding place for the unjustified dead awaiting the final judgment. Those who are in hell will eventually be thrown into the "lake of fire." So, in answer to your questions: Yes, they are two different places. No, hell does not change over time. No, the hell now is the same hell from the OT and the same hell when Christ returns.

Revelation 20:13-15 The sea gave up the dead who were in it, and Death and Hades delivered up the dead who were in them. And they were judged, each one according to his works. 14 Then Death and Hades were cast into the lake of fire. This is the second death. 15 And anyone not found written in the Book of Life was cast into the lake of fire.

Notice that death and hades give up those who are in them to be taken to the final judgment. Notice that hell is cast into the lake of fire. And then those who are not written in the Book of Life are cast into the lake of fire.
