Death and hell were cast into the Lake of Fire

From Rev 20:

What exactly does this mean.

Does this mean that hell is a different place from the Lake of FIre. That hell changes over time? That hell now was not the same hell of the OT and will not be the same hell as when Christ returns?

I have always taken it to mean that "hell" is (and has always been) the holding place for the unjustified dead awaiting the final judgment. Those who are in hell will eventually be thrown into the "lake of fire." So, in answer to your questions: Yes, they are two different places. No, hell does not change over time. No, the hell now is the same hell from the OT and the same hell when Christ returns.

Notice that death and hades give up those who are in them to be taken to the final judgment. Notice that hell is cast into the lake of fire. And then those who are not written in the Book of Life are cast into the lake of fire.
