Does anyone know of such a book by this man (I don't even know his first name)?

I think it is supposed to have a catechism for the family or something in it too. Any help?

I think you may be looking for Daniel Cawdrey's Family Reformation Promoted was reprinted in the old Naphtali Press Anthology of Presbyterian & Reformed Literature v.4 (the one with George Gillespie featured on the white dust jacket). It is out of print but can be picked up used on occasion.
