Here's an entry from the Innerpeffray Library in Scotland: I bet you could get a copy from their microfilm. Quote:

Guide to godlynesse
The title page is engraved and signed: Iohn Payne sculpsit.
"A treatise of securitie" has separate pagination, register, and title page with imprint "London printed by William Stansby. 1622". Formerly also STC 7151.

Part 2 identified as STC 7151 on UMI microfilm reel 1572.
